The PACT Licensed Vocational Nurse (LVN) provides patient-centered, team-based nursing care to Veterans within the Patient Aligned Care Team (PACT) model at a South Santa Rosa VA Outpatient Clinic. The LVN delivers high-quality, coordinated care focused on prevention, chronic disease management, health promotion, and continuity of care. The LVN serves as a key member of the interdisciplinary PACT team, supporting timely access to care, effective communication, and population health management.
