Description
The Intermittent Vocational Nurse (LVN/LPN) provides patient-centered, team-based nursing care to Veterans at the Ukiah VA Outpatient Clinic. The LVN/LPN delivers high-quality, coordinated care focused on prevention, chronic disease management, health promotion, and continuity of care. The LVN/LPN serves as a key member of the interdisciplinary PACT team, supporting timely access to care, effective communication, and population health management.
