You notice a medication or chart detail that does not look right. What do you do?

Read first: a sample answer

Fictional sample answer written by JoRoute to show the shape of a strong answer. Do not copy it: replace every detail with what is true for you.

First I would stop and not give the medication, because patient safety comes before speed. Then I would check the order against the original prescription and the patient's chart. If it still does not look right, I would call the prescriber or the nurse in charge straight away and follow our reporting procedure. For example, a dose ten times higher than usual must be confirmed before anything else. I would document what I found and what was decided, so attention to detail protects the next shift too.

Use only details that are true for you. The benchmark below is fictional.
