Describe a time you explained a care instruction to a patient so they could follow it at home.

In 2023 I discharged a patient who needed to change a wound dressing at home. Communication had to be simple, so I showed her the steps once, then asked her to do it while I watched, the teach-back method. She missed one step, cleaning around the wound, so I wrote the four steps on a card with pictures. After I gave her the card, her dressing mistakes fell to zero, and the wound was healing well at the follow-up visit a week later.

Use only details that are true for you. The benchmark below is fictional.
